Request for Nomination Ballot

Nomination ballots are unique to each Award Category and can be obtained by submitting an email request for the appropriate Award Category Ballot Form. Be sure to refer to the Award Category Definitions prior to submitting your request.

Below are the six (6) award categories reflecting public sector service delivery environments in each Federal, Provincial and Municipal groupings.
