Request for Ballot

NEW for 2014:  This year, the online form requests the majority of  mandatory questions previously asked as part of the Ballot form.  We have included this information ahead of time to expedite the process and to help confirm your eligibility to submit a nomination.  The information must be answered in full in order to download the appropriate category-specific ballot form.

Nomination ballots are unique to each award category and can be obtained by completing the online request form to receive a downloadable PDF file of any of the six (6) category-specific ballots.  Be sure to refer to the award category definitions prior to submitting your request.

A person or team may be nominated in only one group/category, under one ballot form, however, should your initiative be fitting to more than one category, please indicate which other category may be of relevance within the ballot form in the space provided .  Judges reserve the right to re-classify any nomination to a more appropriate group/category, to contact the nominator and/or request supporting materials. Nominations are accepted in both official languages.

Below are the award categories reflecting public sector service delivery environments within all levels of government: Federal, Provincial and Municipal groupings.
